Services
Center Village recognizes
each individual has a unique set of strengths and needs. Our staff works
with everyone's strengths to help people reach and maintain their
independence. At the center of the care is the Individual Program
Plan (IPP). The IPP is held once a year and is designed so that residents,
family members, case workers, legal advocates, and support agencies all
have the opportunity to be active in the planning and goal setting
process. The team then reviews goals every three months and a six month
review is held to discuss and re-set goals.
Adult
Basic Education: This service is provided in conjunction with
Indian Hills Community College with staffing being provided by IHCC. The
class is designed to enhance or stimulate residents' academic skills in
reading, writing, math, history, science and the arts. Comprehensive Adult
Student Assessment System (CASAS) provides a learner centered curriculum,
and serves as an assessment and evaluation system CASAS is used by the ABE
staff to help students ~ and achieve goals. Learning is also assisted by a
computer systems lab which provide hands on exposure and practice to all
consumers.
Activity
Coordination: The activity department at Center Village
coordinates both in house and community activities to meet the residents
social, recreational, physical and religious needs. Some of the various activities include swimming, bowling,
bingo, ethnic days, shopping, picnics, and parties. Monthly resident
council meetings are held so residents have an opportunity to determine
their activities. Such things as special meals and outings are planned at
these meetings. They activity department also plans special monthly meals and
resident birthday menus. The activity department coordinates summer camp
programs for those who wish to attend.
Volunteer
Programs: Center Village promotes volunteerism and encourages
residents to become involved in their community by providing services. We
have coordinated projects with the Villages of Van Buren, the Van Buren
Hospital Day Care Center, and The Department of Natural Resources. We also
encourage residents to help in their home by assisting with dietary tasks,
lawn care, cleaning, monthly mailings, etc.
Transportation:
Center Village offers transportation for all consumers to doctor's
appointments, activities, work programs, hair appointments etc. Center
Village provides this service with the use of two vehicles which were
purchased new in 1995. One of the vehicles is a fifteen passenger van and
the other a station wagon. Center Village also works with the 10-15
Transit System to provide handicap accessible transportation, and other
large group outings 10-15 also runs regular mutes in the area on Monday,
Wednesday, and Friday in which consumers may ride for any personal needs,
mealsite, work programs etc.
Case
Management: Center Village works in conjunction with South East
Iowa Case Management Service to provide enhanced services to consumers.
Case Management is responsible for coordinating services in the community,
screenings, referrals, etc. SEICM is located at P.O. Box 1103, Fairfield,

Vocational
Rehabilitation: Center Village also works with The Division of
Vocational Rehabilitation. This service assists consumers in job
evaluation, assessment, exploration, and serves as a funding source. They

Home
and Community Based Services: Center Village is certified to
provide Supported Community Living through the Home and Community Based
Service waiver. This service is designed to provide consumers with the
supports they need to live in the community. Services may include;
academic services, advocacy services, community skills training services,
personal environment support services, transportation and treatment
services. The consumer works with Center Village and other support
agencies to fulfill his/her needs and reach their life goals. In order to
be eligible for this service consumers must meet admission criteria along
with state regulations. Services will be provided in the consumer's home
or wherever the consumer wishes to reside. Services can be funded up to 24
hours per day, and are based on each individuals needs.
